What it does
A check failed on a Product repo's default branch, or a conformance run failed, with no Change in flight. The QA engineer reads the failure and the Changes merged since the last green commit, decides code, test or release bookkeeping (a version file behind a tag already cut), and opens one issue, or labels the open one that tracks it, which goes through change-quick like any other. Infrared starts it, never twice for one failure: a repeat comments on the open issue, and a failure after a QA-born Change merged goes to a person instead.
When it runs
- On the event
ci.failed. Starts when a check fails on a default branch with no Change in flight.
Steps
Rounded steps are human gates, slanted steps are promotions, and double-edged steps run automatically.
| # | Step | Type | Runs | Required | On failure | Attempts |
|---|---|---|---|---|---|---|

1. triage
Reads the failure, decides code, test or release bookkeeping, and opens one issue a builder can act on, or labels the open one that tracks it.
Skipped when its AgentRole is switched off.
When it hands off to a human
Work an agent cannot finish is dead-lettered to @platform/reviewers. The AgentWorkflowRun shows the step as DeadLettered until a human picks it up.
